Applications Requirements

The first step in admitting your child to Meredith-Dunn School is to contact the Admissions Office at (502) 456-5819, ext. 203. We can help you gather the information listed below:

  • Psychoeducational Report
  • Current Achievement Testing and last report card
  • Completed Parent Statement/Developmental History with attached recent photograph of child
  • Completed Teacher Questionnaire
  • Completed Program Application
  • Financial Aid forms (if applicable)

The admission process and the financial aid process, if applicable, should happen simultaneously. A child must be accepted before he or she will be considered for a financial aid award.

After an application file is complete and an educational match is indicated, a student visit will be scheduled. Once an acceptance letter has been received, parents should return the following:

  • Payment of non-refundable enrollment fee
  • Signed financial contract
  • Signed release for health records
